Schedule your child’s annual eye exam to begin their school year with success
Establishing a relationship with a pediatrician or Family Medicine provider is key to your child’s health, from infancy through young adulthood. Your primary care provider (PCP) will know your child’s medical history from routine check-ups, providing you with informed care and connecting you with our specialists in case of injury or a more serious illness.
We continue to schedule appointments for pediatric wellness visits at a location convenient to you and your family. Schedule an appointment by calling our location nearest you or by logging onto your child’s MyCarle account.
At Carle, your child is our first priority. From a routine checkup, developmental or chronic health conditions, to severe acute illness, our team of specialists work together to care for children and all of their care needs. Our pediatric care teams combine compassion and expertise to evaluate, diagnose and treat children and adolescent patients through individualized care.

During the first two and a half years, we will see your child several times per year. From ages 3-21, well-child visits happen yearly. During well-child visits, you’ll learn about your child’s growth and development. Your child will also receive important immunizations, ensuring they stay well.
School and sports physicals and kindergarten eye exams
Please plan ahead for your child’s school and sports physical or kindergarten eye exam. Illinois requires children entering kindergarten, sixth and ninth grade to have a school physical. Children and teens who participate in school sports also need to have a sports physical on file. Schedule an appointment by calling our location nearest you, or by logging onto your child’s MyCarle account. You can schedule eye examinations by calling (217) 383-3150.
