Inflammatory bowel disease (IBD) includes Crohn’s disease, ulcerative colitis and microscopic colitis. Each one is a chronic condition with a range of severity and symptoms that can change over time. The Carle care team will help you manage your symptoms and improve your quality of life.
IBD Nurse Navigator
The IBD Nurse Navigator is a registered nurse who works closely with the IBD care team and provides clinical expertise and program management for you under a modified medical home. Your Nurse Navigator will work closely with your doctor and the nursing team in order to assist in your plan of care. For newly diagnosed patients, this nurse is a great resource and someone to call and ask questions, even if you aren’t sure with whom you need to speak.
Nutrition Support
Adequate nutrition is essential and often challenging for individuals with IBD, since the condition can make it difficult for you to absorb nutrients. Through nutrition consults, a staff dietitian can evaluate your nutrition levels and make recommendations.
Support Groups
Facilitated by Registered Nurses and Nurse Practitioners, we offer two support groups for patients and family members to learn more about IBD and living with an ostomy, as well as to connect with people who share similar situations. All support groups are free of charge.
